Early Assessment: Severe Sepsis Triage Screening Tool
Does the patient fit either of the following?
High risk patient* + tachycardia for age
Any patient with T ≥100.4°F (38°C) or ≤96.8°F (36°C) and either: Abnormal heart rate, Abnormal blood pressure for age,
Clinically ill (altered mental status, ill appearing, poor perfusion)
If yes, then alert a qualified provider for rapid assessment.
*High risk is defined as either:
Immune compromised: active oncology patient, on a prolonged steroid course, on immune suppressive medications , neuromuscular disease, sickle cell disease, asplenia, transplant recipient, other immune suppressive condition; or,
